Health AssessmentNorth Carolina Law G.S. 130A-440 requires that parents or guardians of any child entering kindergarten in the public schools must secure a health assessment for their child. The health assessment shall be made no more than 12 months prior to the date of school entry.
What Needs to Be Included?
The health assessment shall include a medical history and physical examination with screening for vision and hearing and, if appropriate, testing for anemia and tuberculosis. The school must be provided with a copy of the immunization record for all students entering public school for the first time. It is important that the immunization record is received before the first day of school.
